The LK201 keyboard that came with my VT220 really isn't very good for
Unix use. If anyone's got a LK421 they'd consider parting with, please
email me off-list. Or, if it's possible to adapt another type of
keyboard for use with the VT-220, let's discuss it on-list! :)

As far as I know the LK201, LK401, etc all use 4800 baud 8N1 at +/-
5V. Standard serial keyboards are 9600 baud, but with a small
microcontroller it shouldn't be too difficult to get working.

what the OP was asking. What's the difference?

That thing goes the other way around.

What's the difference?
The former connects a PS/2 keyboard to a VT320 terminal.
The later connects a LK201/LK401 keyboard to a PeeCee.
